1. We respect your privacyPelvic Care Physiotherapy respects your right to privacy and is committed to safeguarding the privacy of our patients and website visitors. We adhere to the National Privacy Principles (NPPs) contained in the Privacy Act 1988. A copy of the NPPs may be obtained from The Office of the Federal Privacy Commissioner at www.privacy.gov.au. This policy sets out how we collect and treat your personal information. “Personal Information” is information we hold which is identifiable as being about you. 2. What Personal Information do we collect and how?The Personal Information we collect includes: names, date of birth, addresses, contact details, medical history, medications, allergies, adverse events, social history, family history and financial or credit card information. This Personal Information is obtained in many ways including when you communicate with us through our website, call to make an enquiry about our services, or when information is provided to us by a third party, such as your referring practitioner. 3. How do we use your Personal Information?When you register as a patient of Pelvic Care Physiotherapy, you provide consent for our physiotherapists and any practice staff to access and use your Personal Information so that we can provide you with a high-quality service. Only staff who need to see your Personal Information will have access to it. The information we collect allows us: - to develop therapy services to support the achievement of your goals;- to invoice and process any fees payable in relation to the services rendered;- to contact you for follow up purposes;- to verify and update information held by us;- to review, develop and improve our services, as well as our business, operational and IT processes and systems;- to train staff- to resolve any complaints and issues;- to comply with our legal or regulatory obligations; Where reasonable and practicable to do so, we will only collect your Personal Information from you. However, in certain circumstances we may be provided with information by third parties, for instance your referring doctor or health care team. In this case we will take reasonable steps to ensure that you are made aware of the information given to us by the third party. We will protect this information as set out by this policy. We do not guarantee website links or policy of authorised third parties. 4. Disclosure of your Personal InformationYour Personal Information may be disclosed in several circumstances, such as: - to third parties (whereby you consent to the use or disclosure)- where required or authorised by law- when it is necessary to lessen or prevent a serious threat to a patient’s life, health, safety; or public health or safety; or it is impractical to obtain the patient’s consent- to establish, exercise or defend an equitable claim- for the purpose of confidential dispute resolution process 5. Security of your Personal InformationOur practice stores all Personal Information securely in protected information systems that reasonably protect it from misuse, loss and from unauthorized access, modification or disclosure. Any paper records are digitised and securely destroyed.
6. Access and correction of Personal Information To ensure we can continue to provide quality services to you our practice will take reasonable steps to correct and update your Personal Information. You may also request that we correct or update this information.You have the right to request access to your Personal Information. However, our practice may occasionally need to deny access to information in accordance with the exemptions contained in the Privacy Act. If you wish to access your Personal Information, please contact us in writing at info@pelviccare.com.au. In order to protect your Personal Information we may require identification from you before releasing the requested information.Pelvic Care Physiotherapy may charge an administrative fee for providing a copy of your Personal Information.
7. Privacy Policy complaints and questionsWe take complaints and concerns regarding privacy seriously. You should express any privacy concerns you may have in writing to info@pelviccare.com.au. We will then attempt to resolve it with you.You may also contact the Office of the Australian Information Commissioner (OAIC). 8. Privacy policy changesThis policy will be reviewed periodically and updates to this policy will be made available on our website.
9. Website visitorsCertain visitors to our website chose to interact with Pelvic Care Physiotherapy in ways that require gathering of Personal Information. Personal Information collected as part of making an appointment to see a Pelvic Care Physiotherapist is treated and handled as described in the above privacy policy.Pelvic Care Physiotherapy collects such information only insofar as is necessary or appropriate to fulfil the purpose of the visitor’s interaction with our practice.
